Welcome to 魅色天堂 as a new student! Below you will find helpful information to become an established patient with the Student Health Center. These steps will allow us to serve you more quickly and effectively. 

Step 1: 

• Access the . This is connected with My魅色天堂. If you are not logged into My魅色天堂, you will be asked to log-in first. In the My Forms section of the My Health Portal, you will find the Patient Registration Form and the No-Show & Late Cancellation Policy Acknowledgement Form. Both of these forms can be completed online. This will complete your first step. 

Step 2: 

• Next click on the Document Upload tab, select Document Type, select SHC Insurance Card, browse for file, Upload, select Save. This can be a picture of your insurance card (front and back).
